Please keep in mind that Maggiore Family Eye Care works for you, not your insurance company, which means that you are ultimately responsible for all payments. Listed below are some of the insurances we accept, if you do not see yours here, you can call the office to verify your plan. What Is Covered?
The eye care services that are covered by your insurance provider are not decided by our office. They are typically determined by a contractual agreement between the insurance company and an employer. Every insurance provider has several plans that cover different services. It is absolutely your obligation to know the benefits and limitations of your insurance plan. Some insurance plans require referrals and others require that you only use doctors on a list. You should be familiar with your insurance plan coverage and the necessary fulfillment requirements before making your appointment. Many typical medical plans (indemnity, HMO, PPO, IPO) specifically “exclude refraction” and contact lens related exams. This means that the determination of contact lens or eyeglass prescriptions are not a covered service. If you decide to have our office evaluate the fit of your contract lenses and/or change your prescriptions, there will be an out-of-pocket fee involved.
